| (1) |
An application for designation as DUE shall be submitted to the Director within 60 days of having complied with the requirements as provided in regulation 71.04.2. |
| (2) |
An application referred to in subregulation (1) shall be in a prescribed form accompanied by— |
| (a) |
a signed code of conduct; |
| (b) |
certified copy of two most recent pages of an applicant's flying logbook, including an hour summary indicating compliance with the hour-requirements for a designation sought; |
| (c) |
report indicating successful completion of two mock tests or checks, signed by an approved DRE as specified in this Subpart; |
| (d) |
report indicating a pass in initial DRE theoretical knowledge examinations conducted by the Authority; and |
| (e) |
proof of payment of the applicable fee, as prescribed in Part 187. |
| (3) |
An application that contains false or misleading information, including in any supporting documentation shall be disqualified. |
| (4) |
If any false or misleading information comes to the attention of the Director subsequent to the approval for a DUE designation, the Director may withdraw a designation in writing. |
| (5) |
The Director may institute criminal proceedings in terms of Part 185 where an applicant is suspected or found to have falsified information. |
