An application for the issuing of a supplemental type certificate shall be—
| (a) | made in the prescribed form and manner; and |
| (b) | accompanied by— |
| (i) | the appropriate fee as prescribed in Part 187; and |
| (ii) | proof of compliance with the provisions of regulation 21.05.1. |
