| (1) |
An application for the issuing of an ATS licence shall be made to the Director in the appropriate prescribed form. |
| (2) |
An application referred to in subregulation (1) shall be accompanied by— |
| (a) |
an original or certified copy of identity document of an applicant showing; |
| (b) |
proof of age of an applicant if not indicated in the identity document; |
| (c) |
appropriate medical certificate issued in terms of Part 67; |
| (d) |
appropriate certificate of competency as prescribed in Document SA-CATS 65, which has been signed by a rating assessment examiner; |
| (e) |
appropriate fee as prescribed in Part 187; and |
| (f) |
two recent passport size photographs of the applicant. |
