An application for the issuing of a post maintenance test flight rating or a test pilot rating shall be made to the Director or the organisation designated for the purpose in terms of Part 149, as the case may be, on the appropriate form and accompanied by—
| (a) | the appropriate fee prescribed in Part 187; and |
| (b) | a copy of his or her logbook, duly summarised, showing the required fying experience. |
