(1)	If a certificate issued under this Part is lost, stolen, damaged or destroyed, the holder thereof, or an AMO approved under Part 145 which is responsible for the servicing and maintenance of the aircraft, may apply to the Director for the issue of a duplicate certificate.

(2)	An application referred to in subregulation (1) shall be—

(a)	made in the prescribed form; and

(b)	accompanied by the appropriate fee as prescribed in Part 187.

(3)	A duplicate of the certificate shall be issued in the prescribed form.
