| (1) | The Director shall issue a SPL in the appropriate prescribed form, as by the Director, if the applicant complies with the requirements referred to in regulation 61.02.2. |
| (2) | Upon receipt of the SPL, the holder must immediately affix his or her signature thereon in ink in the space provided for such purpose. |
