An application for the issuing of a design organisation approval or an amendment
thereof, shall be—
| (a) | made in the prescribed form and manner; and |
| (b) | accompanied by— |
| (i) | the appropriate fee as prescribed in Part 187; |
| (ii) | the manual of procedure referred to in regulation 147.02.2; and |
| (iii) | the terms of approval requested to be issued under regulation 147.02.8, for which application is being made. |
