An application for a certificate of approval as screening organisation and renewal of such a certificate of approval shall—
| (a) | be made to the Director in the prescribed form; |
| (b) | be accompanied by duplicate copies of operations manual referred to in regulation 101.03.1 (2); and |
| (c) | be accompanied by appropriate fee or fees as prescribed in Part 187. |
