An application for the issuing of an AMO approval, or an amendment thereof, shall be—
| (a) | made to the Director in the appropriate from as prescribed by the Director; and |
| (b) | accompanied by— |
| (i) | the appropriate fee as prescribed in Part 187; and |
| (ii) | the manual of procedure referred to in regulation 145.02.1. |
