Class Licensing Processes and Procedures Regulations (as amended), 2010

4. The prescribed forms

 

(1) The prescribed forms in these regulations are as follows:
(a) Form A: Registration of a class licence (Regulation 7(1));
(b) Form B: Registration of a class licence to provide a Community Broadcasting Service and (Regulation 7(2) and to provide a Special Event and Temporary Community Television Broadcasting Services (Regulation 13A);

(c) Form D: Application to amend a class licence (Regulation 8(1));
(d) Form  F: Notice to renew a class licence (Regulation 9(1));
(e) Form H: Application to transfer a class licence (Regulation 10);
(f) Form K: Notice of surrender of a class licence (Regulation 11));
(g) Form L: Notice of change of information in respect of a class licence (Regulation 12);
(h) Form M: Licence exemption services and networks (Regulation 13);
(i) Form N:  Registration for class licence to provide a Special Event and Temporary Community Television Broadcasting Services (Regulation 13A); and

 

(2) Forms referred to in regulation 4(1) must be used for any registration, notice or application and may be;
(a) downloaded from the Authority's website;
(b) obtained from the Authority at its offices.
