A Licensee must submit written notice to the Authority within fourteen (14) days of occurrence of the following changes: |
| (a) |
the name of the Licensee; |
| (b) |
contact details including the contact persons (e.g. telephone, fax, cell number and email); |
| (c) |
Board of Directors or Trustees; |
| (d) |
Principal place of business; and |
