Form C: Application to amend an Individual Licence (Regulation 9); |

Form E: Application to renew an individual Licence (Regulation 10); |
| (c) |
Form G: Application to transfer ownership and/or control of an individual Licence (Regulation 11); |

| (d) |
Form I: Notice of surrender of an individual Licence (Regulation 13); |
| (e) |
form J: Application for a special temporary authorisation (Regulation 14); |

Forms referred to in regulation 4(1) must be used for any application or notice and may be: |
| (a) |
downloaded from the Authority's website; |
| (b) |
obtained from the Authority at its offices. |
