| (1) | The applicant for national listing must submit a notice for the withdrawal of an application to the Registrar on the form obtainable from the office of the Registrar. |
| (2) | A duly completed and signed form must be sent electronically, mailed, couriered or hand delivered to the office of the Registrar. |
