The following incidents must be reported by staff members to the manager of the child and youth care centre within an hour of the discovery or reporting of the incident:
| (a) |
Removal or any attempted removal of a child from the child and youth care centre or programme by anyone who is not permitted to do so; |
| (b) |
any situation in which restraint, isolation, or prohibited behaviour |
| (c) |
management measures are used; |
| (d) |
accident or illness requiring medical attention or hospitalisation; |
| (e) |
allegations of physical, psychological, emotional, sexual or verbal abuse; |
| (f) |
absence of a child or young person from the centre without permission, or as otherwise agreed within the individual development plan; |
| (g) |
interventions by security personnel or the South African Police Service; |
| (h) |
the death or injury or a child; |
| (i) |
any criminal charge or conviction of a service provider, volunteer or other adult involved with the centre; |
| (j) |
any substance abuse by a service provider while on duty or when he or she arrives on duty under the influence of alcohol or drugs or he or she deals in drugs; |
| (k) |
any strike by workers at the centre; or |
| (l) |
any other unusual circumstances that are likely to affect the safety or well-being of any child at the centre. |
