A request by a provincial department of social development or a designated child protection organisation for the removal of an alleged offender from his or her home or from the place where he or she resides as contemplated in section 110(7)(b) of the Act, must—
| (a) | contain particulars regarding the alleged offender; and |
| (b) | in writing in a form identical to Form 24. |
