| (1) | A parent or guardian who wishes to withdraw the consent must to so in writing, in a form identical to Form 64, in the presence of any presiding officer within 60 days of such consent. |
| (2) | A child who wishes to withdraw the consent must do so in writing, in a form identical to Form 65, in the presence of any presiding officer with 60 days of such consent. |
