(1) |
An inquiry by anyone in terms of section 117 of the Act to establish whether or not his or her name appears in Part A of the National Child Protection Register, must be— |
(a) |
directed to the Director-General; |
(b) |
contained in a sealed envelope marked confidential; |
(c) |
accompanied by a certified copy of the affected person's birth certificate, identity document or passport; and |
(d) |
in a form identical to Form 26. |
(2) |
If the person making the inquiry in terms of sub-regulation (1) is a child below the age of 12 years, such child must be assisted in making the inquiry by his or her parent, guardian, care-giver or by a designated social worker, unless it is demonstrated to the satisfaction of the Director-General that the child is of sufficient maturity to make the inquiry on his or her own. |

In furnishing reasons for the inclusion of an affected person's name in Part A of the National Child Protection Register as contemplated in sub-regulation (1)(d), the Director-General must provide— |
(a) |
particulars regarding the date and place of the incident or act that led to the inclusion of the affected person's name in Part A of the National Child Protection Register; and |

(b) |
a brief description of the incident or act that led to the inclusion. |
