| (1) | An application for a copy of any classification decision of a classification committee shall be made on Form BOARD/CP, attached as Annexure "M", and submitted to the Board with proof of payment of the prescribed fee. |
| (2) | An application for a copy of a decision contemplated in subregulation (1) shall be made by the person who initially submitted a film, game or publication for classification or his or her nominee. |
