| (1) | A notice of change of information in a licence must be submitted in the format as set out in Form L. |
| (2) | A licensee must submit the notice within seven (7) days of the change occurring where: |
| (a) | name of contact details of the licensee changes; or |
| (b) | nature of the service/s provided in terms of the licence change. |
