| (1) | An application for registration in terms of clause 5 shall be made on the application form that is available from SAGIS. |
| (2) | The application form shall be completed on a computer or in ink by a person who is duly authorised and it shall be accompanied by the corroborating documentation as specified in the application form. |
| (3) | The application form shall be submitted to SAGIS at any of the addresses furnished on the official application forms. |
