An owner of a registered dental laboratory shall, within 30 days after any of the following has occurred, advise the registrar thereof by registered post:
| (a) |
A change of directors and shareholders, where the owner is a body corporate. The full name, address and occupation of the person or persons concerned shall be stated. |
| (b) |
The retirement or joining of a partner, where the owner is a partnership. The full name, address and occupation of the person concerned shall be stated. |
| (c) |
A change of the person supervising the laboratory in a personal capacity in terms of section 29(4) of the Act: |
| (i) |
The new supervisor's qualifications shall be stated as well as whether he is or was a dental technician contractor. |
| (ii) |
Should such supervisor not be or have been a dental technician contractor, the provisions of regulation 3(2) shall mutatis mutandis be applicable. |
| (d) |
Closing of the laboratory for dental work. |
