The record of proceedings referred to in section 30 (9) of the Act must comprise—

10.1.1

the form setting out the complainant's grievance against the respondent, contemplated in section 28 (1);

10.1.2

the notice of mediation contemplated in section 29 (3) (a);

10.1.3

the certificate contemplated in section 29 (4) (b) (i);

10.1.4

any compliance notices issued to the respondent in terms of section 26;

10.1.5

the notice of adjudication contemplated in section 30 (1);

10.1.6

a transcript of the adjudication;

10.1.7

the adjudicator's written reasons for either upholding or dismissing the complaint, and written order; and

10.1.8

the notice of appeal referred to in section 31 (1).
