National Licences
| 1) | An application for a national licence in terms of section 38(3) shall be in writing in the relevant form of Form NGB 5/1, and must be accompanied by the fee stipulated in Schedule 1 in respect of such category of national licence. |
| 2) | A notification by a provincial licensing authority to issue a national licence certificate shall be in the form of Form NGB 5/2. |
