| 1) | An application under section 148A of the Act to omit information required by section 148 must: |
| a) | identify the information concerned; and |
| b) | be supported by a full motivation. |
| 2) | The prescribed fee set out in item 58 of Schedule 1 shall be payable in respect of such application. |
