An application by a voluntary association for renewal of its recognition as a voluntary association in terms of section 26(7) of the Act shall—
| (a) |
be in writing on the letterhead of the voluntary association; |
| (b) |
be signed by the president of the voluntary association or his/her nominee; |
| (i) |
that the voluntary association complies with the requirements determined by the council in terms of section 15(d) of the Act; and |
| (ii) |
that it will at all times comply with the said requirements; and |
| (i) |
the current constitution of the voluntary association; |
| (ii) |
a list containing the names and postal and e-mail addresses of the committee and its office-bearers; and |
| (e) |
a list of the voluntary association’s members and the category of membership of each member. |
