| (1) | An application for written authorisation to replace a vehicle temporarily in terms of section 74(1) of the Act must be lodged by submitting the completed application form shown in Schedule 1 to the relevant regulatory entity, together with the information and documents specified in the form and the application fee specified in Schedule 2. |
| (2) | Such authorisation must be in the form shown in Schedule 1. |
