| (1) |
Whenever the certification committee has expressed a finding in accordance with the provisions of this Act, the chairman or a person authorized thereto in writing by him, shall issue a certificate in the prescribed form setting out such finding and containing such information as may be necessary for the purposes of this Act, and shall within ten days as from the date on which the finding was expressed, cause copies of such certificate to be sent— |
| (b) |
if the person to whom the certificate relates is still employed at a mine or works, to the owner of such mine or works; |
| (c) |
to the person to whom it relates, or if it relates to a deceased person, to the dependants, if any, of the deceased; |

| (e) |
if it is a finding of tuberculosis, to the local authority in whose area the person is to whom the certificate relates. |
| (2) |
The chairman of the certification committee shall keep a register of all the findings of that committee. |
