| (1) | Any person referred to in section 3 who has attained the age of 16 years shall in the prescribed form and within the prescribed period apply for an identity card. |
| (2) | Two copies of the photograph referred to in section 9, and the fingerprints referred to in section 10, shall accompany the application made under this section. |
