Part B of the Register must be a record of persons found in terms of section 120 to be unsuitable to work with children, and must reflect—
| (a) |
the full names, surname, last known physical address and identification number of the person; |
| (b) |
the fingerprints of the person, if available; |
| (c) |
a photograph of the person, if available; |
| (d) |
a brief summary of the reasons why the person was found to be unsuitable to work with children; |
| (e) |
in the case of a person convicted of an offence against a child, particulars of the offence of which he or she has been convicted, the sentence imposed, the date of conviction and the case number; and |
