The name of the mine.

The mine code number.

1.1.3.

The heading: Mandatory Code of Practice for Road and Rail Safety Management in the South African Mining Industry.

1.1.4.

A statement to the effect that the COP was drawn up in accordance with the guideline with reference number DMPR 16/3/2/2-B5 issued by the CIOM.

1.1.5.

The mine reference number for the COP.

1.1.6.

The effective date of the COP.

1.1.7.

The revision dates of the COP (if applicable).
