6.	Fax number of the applicant;

7.	Email address of the applicant; and

8.	Details of the mandated representative of the applicant, including:

(a)	designation,

(b)	family name,

(c)	first name,

(d)	telephone number,

(e)	fax number, and

(f)	email address.
