Traditional Health Practitioners Act, 2007 (Act No. 22 of 2007)Chapter 3 : Registrar, Staff of Registrar and Registration Procedures24. Issue of duplicate registration certificate, certificate of status and extract from register or certificate |
(1) | The registrar may, on application by a registered traditional health practitioner, issue a duplicate certificate of registration if the applicant— |
(a) | provides proof of his or her identity to the satisfaction of the registrar; |
(b) | provides an affidavit in which he or she confirms that the certificate of registration has been lost or destroyed; and |
(c) | pays the prescribed fee determined by the Council. |
(2) | The registrar may, upon payment of the prescribed fee, issue to any registered person a certificate of status containing— |
(a) | particulars of such person's registration; and |
(b) | a statement to the effect that— |
(i) | the said person is not disqualified from practising his or her occupation; and |
(ii) | no disciplinary steps are pending against him or her in terms of this Act. |
(3) | The registrar may issue a certified extract from the register or a certificate contemplated in subsection (2) under his or her hand to any person upon payment of the prescribed fee. |
(4) | A certificate may be issued subject to certain conditions imposed by the Council and such conditions must be indicated on the certificate. |
